I have answered the screening questions honestly and disclosed relevant allergies, conditions, medications and prior reactions.
I understand results vary with natural lash condition, growth cycle, aftercare and previous chemical services. No specific result or duration is guaranteed.
I understand temporary redness, watering, irritation, dryness, uneven lift, over-processing, lash breakage, tint staining or an allergic reaction may occur.
I will remove contact lenses before the service and keep my eyes closed and still throughout. I will immediately report any burning, stinging or discomfort.
I understand a patch test may reduce risk but cannot guarantee that a reaction will not occur. I may request a patch test before treatment.
I authorise the technician to stop, modify or decline the service if it is considered unsafe or unsuitable. I understand fees may still apply for time and products used.
I agree to follow the written aftercare instructions and to seek medical attention if symptoms are severe, persistent or affect my vision.
I voluntarily consent to the Korean lash lift and tint service and release the studio and technician from claims arising from normal, disclosed risks, except where prohibited by law.
